Legal Release: I hereby do indemnify and hold harmless Melissa Heltman, Heart & Sole Skippers Jump Rope Team, Teal Star Enrichment, their volunteers, assistants, assigns, agents, any facilities used by same, their owners, agents, employees, volunteers and others involved in the events from any and all liability, excluding intentional acts or intentional torts. Furthermore, I will not take any legal action against Melissa Heltman or any coach affiliates of the Heart & Sole Skippers team, Teal Star Enrichment or the venue in which an emergency may occur. I understand that my child(ren), listed above, is participating in this activity at their own risk. By signing below, I release all above mentioned parties of all legal responsibility for any negligence of any kind excluding intentional or criminal acts.

Media Consent: I hereby consent to the participation in interviews, the use of quotes, and the taking of photographs, audio recordings, movies, or video of the participant(s) named above by Heart & Sole Skippers, Teal Star Enrichment Coaches and Staff. I also grant the right to edit, use, and reuse for purposes including use in print, on the internet, and all other forms of media. I understand that the images &/or video’s may be released to the public by the news media or by Heart & Sole Skippers, Teal Star Enrichment Coaches and Staff. I further understand that the name of the minor may be used. I hereby release Heart & Sole Skippers, Teal Star Enrichment Coaches and Staff and its agents from all claims, demands, and liabilities whatsoever in connection with the above.
